Your first visit, made simple.
Oklahoma is a direct-access state, which means you can come straight to PT for evaluation and up to 30 days of treatment without a physician referral. If you have a referring provider, we'll coordinate with them; if not, we'll get started anyway.
Plan to arrive about 30 minutes early on your first visit so we have time to review paperwork together.

It is important to keep us informed of any changes that occur such as a change in insurance, address change, phone number change, a change in physician or any other pertinent information.
Parking & access
We're at 6767 S. Yale Avenue, Suite B. Parking is free and steps from the entrance. We have a wheelchair ramp into the building, and if you let us know ahead of time, we're happy to help you in.

Straightforward billing.
We bill insurance directly. After insurance processes your claim, you'll receive a statement for any patient-responsibility balance. Self-pay rates are available — call us for a quote.

Good Faith Estimate
Under the No Surprises Act, you have the right to receive a Good Faith Estimate of your expected charges. If you'd like one, ask the front desk or email billing and we'll get it back to you promptly.
