Our pledge regarding your health information
Physical Therapy of Tulsa is committed to protecting medical information about you. We are required by law to maintain the privacy of your protected health information (PHI), provide you with this Notice of our legal duties and privacy practices, follow the terms of the Notice currently in effect, and notify you in the event of a breach of unsecured PHI.
How we may use and disclose your health information
For treatment
We may use and disclose your PHI to provide, coordinate, or manage your physical therapy care — including consultation with referring physicians, specialists, and other providers involved in your treatment.
For payment
We may use and disclose your PHI to bill and collect payment for the services we provide. This may include sharing information with your health insurance plan to verify coverage, obtain prior authorization, and submit claims.
For health care operations
We may use and disclose your PHI for activities necessary to run our practice, such as quality assessment, staff training, accreditation, licensing, business management, and general administrative activities.
Other permitted uses and disclosures
Required by law: when disclosure is required by federal, state, or local law.
Public health activities: reporting to public health authorities to prevent disease, injury, or disability.
Health oversight: activities authorized by law, including audits, investigations, and inspections.
Judicial and administrative proceedings: in response to a court order, subpoena, or warrant.
Workers' compensation: as authorized by, and to the extent necessary to comply with, workers' compensation laws.
Serious threats to health or safety: to prevent or lessen a serious threat to a person or the public.
Uses and disclosures that require your written authorization
Other uses and disclosures of your PHI not described in this Notice will be made only with your written authorization. You may revoke an authorization at any time, in writing, except to the extent we have already acted in reliance on it.
Your rights regarding your health information
Right to inspect and copy your PHI in our designated record set.
Right to amend PHI you believe is incorrect or incomplete.
Right to an accounting of disclosures we have made of your PHI.
Right to request restrictions on certain uses or disclosures.
Right to request confidential communications at an alternate address or by alternate means.
Right to a paper copy of this Notice.
Right to be notified of a breach of your unsecured PHI.

Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
Changes to this Notice
We reserve the right to change this Notice and to make the new terms effective for all PHI we maintain. The current version is always available at our clinic and on this website.
