When you come for your first visit to Physical Therapy of Tulsa plan on spending at least one hour and bring:
- Doctor’s order
- Driver’s license
- Insurance card
- Insurance co-pay (if applicable)
- Shorts or T-shirt (to allow access to area being treated. Shorts and T-shirts are available if needed)
It is important to keep us informed of any changes that occur such as a change in insurance, address change, phone number change, a change in physician or any other pertinent information.
